I don't quite understand. Is this that far off from a bear crawl? Does the revers and side to side movement add a lot to it?

excellent movement, and if you wanna kick it up a notch still. try it up some stairs, or a hill. people will think you're mental but if you're doing them, you could probably kick their ass anyway.

To put that into perspective, that'd be like asking "is a front squat far off from a back squat?" ;)

Similar but dissimilar movement patterns. Similar but dissimilar muscle recruitment.

My quads are on fire after cat crawls, due to the bent-knee position through-out... and the fact that one has to fire the quads and calves hard at the start of each step.

Conversely, the bear crawl is less about quads, and more about the hamstrings trying to fire in an extreme stretch position.

It does.
From both a muscle recruit point of view as well as a coordination-specific point of view.

I tried this today. Crazy, I never guessed how hard moving in reverse would be. After about the 4th set I could barely lift my legs right.
I really liked the slow, deliberate pace of it.

Sorta like the rest-pause version of the bear crawl.

Definitely a new training tool for me.

Next we've got to get you moving diagonally from left to right.

...maybe get a training partner to call the direction so your body doesn't fall into a comfortable pattern.

And, after that, we can add a weighted vest.
